What to Bring to the Illinois DMV for Your Permit Test

Don't get sent home. Here's the complete checklist — documents, fees, forms, and what to leave at home.

Before You Go

  • Schedule an appointment online to skip the long lines
  • Print out the 'Document Checklist' from the SOS website to ensure you have everything
  • Arrive at least 15 minutes early to check in
  • Bring a study guide or flashcards for the permit test
  • Make sure your eyes are rested; you will take a vision test immediately
  • Double-check that your parent/guardian knows they must come with you
  • Verify your address on all documents matches exactly

Required Documents

Proof of Identity (Primary Document)

  • U.S. Passport or Passport Card
  • Certified U.S. Birth Certificate
  • Permanent Resident Card (Green Card)
  • Consular Report of Birth Abroad
  • Certificate of Naturalization or Citizenship

You must bring the original document; photocopies or digital photos on your phone will not be accepted. If you are applying for a REAL ID, ensure your birth certificate is a certified copy issued by the state or county.

Proof of Social Security Number

  • Social Security Card
  • W-2 Form with full SSN
  • Paystub with full SSN
  • SSA-1099 Form
  • Non-SSA-1099 Form

Illinois requires proof of your SSN. If you do not have a card, you must bring a letter from the Social Security Administration stating you have been assigned a number. If you have never been issued an SSN, you must bring a letter from the SSA stating you are ineligible.

Proof of Illinois Residency

Bring 2 of these:

  • Utility bill (electric, gas, water, sewer) issued within 90 days
  • Bank or credit card statement issued within 90 days
  • School report card or transcript with current address
  • Parent's lease or mortgage statement (if living with them)
  • Illinois vehicle registration or title

For a standard license, one document is usually enough, but for a REAL ID (which is recommended), you generally need two different documents showing your name and current Illinois address. The address on these must match exactly.

Parental / Guardian Consent (applicants under 18)

  • Parent or legal guardian must appear in person with you at the office
  • Parent must sign the application form in front of the examiner
  • Parent must provide their own proof of identity and residency
  • If parents are divorced, the custodial parent usually signs, but bring custody papers if there's a dispute
  • Legal guardians must bring court documents proving guardianship

If a parent cannot appear in person, they must sign a notarized affidavit of consent (Form DL-180TD) and mail it to the SOS office ahead of time, but appearing in person is the fastest and most reliable method to avoid delays.

Fees & Payment

$20

This $20 fee covers the application for the learner's permit. It is valid for two years. If you fail the vision or written test, there is no additional fee to retake it on the same day, but you may need to wait 24 hours before retaking the written test.

Forms

Application for Driver's License or Identification Card (Form DL-44)

Available at: Available at the Secretary of State office or downloadable from ilos.gov

Do not sign this form until you are standing in front of the examiner. If you fill it out online via the 'Online Services' portal, you can skip the paper form, but you still need to bring all your physical documents.

What NOT to Bring

  • Your cell phone (use it only for emergencies, no texting during the test)
  • Large bags or backpacks (storage is limited)
  • Friends or siblings (they cannot wait inside the testing area)
  • Food or drinks (not allowed in the testing room)
  • Your own pen (the office provides them, but bring one just in case)
  • Pets (service animals only)
  • Expensive jewelry or watches (keep it simple)

At the Office

  • You will take a vision test first; if you wear glasses/contacts, bring them
  • The written test is computer-based; read questions carefully and use the 'help' button if needed
  • If you fail the written test, you can usually retake it the next day at no extra cost
  • Once you pass, you will get an interim permit immediately; the plastic card arrives by mail in 2-3 weeks
  • The interim permit is valid for driving immediately, but you must follow all learner's permit rules (supervised driving, no night driving, etc.)
