What to Bring to the Iowa DMV for Your Permit Test

Don't get sent home. Here's the complete checklist — documents, fees, forms, and what to leave at home.

Before You Go

  • Call ahead to check if you need an appointment or if it's first-come, first-served
  • Have your parent or guardian clear their schedule to come with you
  • Bring your original birth certificate and social security card (no copies)
  • Study the Iowa Driver's Manual online before you go
  • Arrive at least 15 minutes early to fill out paperwork
  • Make sure your parent has a valid photo ID with them
  • Get a good night's sleep so you're sharp for the vision and written tests

Required Documents

Proof of Identity (Primary Document)

  • Valid U.S. Passport
  • Certified U.S. Birth Certificate
  • Permanent Resident Card (Green Card)
  • Consular Report of Birth Abroad
  • Certificate of Naturalization

Bring the original document only; photocopies are not accepted. If you want a REAL ID, you must bring your primary ID plus two proofs of residency.

Proof of Social Security Number

  • Social Security Card
  • W-2 Form with full SSN
  • Paystub with full SSN
  • SSA-1099 Form
  • Non-SSA-1099 Form

Iowa requires proof of your SSN. If you don't have a card, bring a document from the SSA showing your number. If you are not eligible for an SSN, you must bring a letter from the SSA stating ineligibility.

Proof of Iowa Residency

Bring 2 of these:

  • Utility bill (electric, water, gas) in parent's name
  • Bank statement
  • School report card or transcript
  • Parent's mortgage or lease agreement
  • Iowa voter registration card

For a standard license, one proof is usually enough, but for a REAL ID, you strictly need two different documents showing your Iowa address. The address must match what you put on the application.

Parental / Guardian Consent (applicants under 18)

  • Parent or legal guardian must sign the application in front of the clerk
  • Parent must provide their own valid ID
  • If parents are divorced, the custodial parent must sign
  • Legal guardians must bring court documents proving guardianship
  • Foster parents must bring specific agency documentation

The parent or guardian MUST be present with you at the office to sign. They cannot just sign the form at home and have you bring it in.

Fees & Payment

$8

This $8 fee covers the instruction permit. If you fail the vision or written test, there may be a small fee to retake it, but the permit fee itself is for the card issuance.

Forms

Application for Driver's License or Non-Operator ID (Form 430003)

Available at: Iowa DOT office or downloadable from iowadot.gov

Do not sign this form until you are standing in front of the clerk; they need to watch you sign it.

What NOT to Bring

  • Large backpacks or heavy bags (lockers are limited)
  • Your cell phone (you'll need to put it away during the test)
  • Friends or siblings (they can't wait inside the testing area)
  • Food or drinks (no eating in the office)
  • Your own pen (they will provide one)
  • Pets (unless it's a service animal)
  • Photocopies of your documents (must be originals)

At the Office

  • You will take a vision screening first to make sure you can see the signs
  • The written test is computer-based, so listen carefully to the instructions
  • If you fail the written test, you can usually retake it the same day for a small fee
  • Once you pass, you'll get an interim paper permit immediately; the plastic card comes in the mail
  • Your parent must sign the application with you at the counter, not before
