What to Bring to the Missouri DMV for Your Permit Test

Don't get sent home. Here's the complete checklist — documents, fees, forms, and what to leave at home.

Before You Go

  • Check the DOR website for any specific office hours or holiday closures
  • Call ahead to see if you need an appointment (some offices require them)
  • Have your parent/guardian ready to go with you
  • Bring your study guide for the permit test
  • Arrive at least 15 minutes early to get your number
  • Double-check that all documents are originals, not copies
  • Make sure your parent has their own ID ready

Required Documents

Proof of Identity (Primary Document)

  • U.S. Passport
  • Certified U.S. Birth Certificate
  • Permanent Resident Card
  • Certificate of Naturalization
  • Valid Missouri Driver License or ID Card

You must bring the original document; photocopies are not accepted for the primary identity proof.

Proof of Social Security Number

  • Social Security Card
  • W-2 Form
  • Pay Stub with full SSN
  • SSA-1099 Form
  • Non-SSA-1099 Form

Missouri requires proof of your SSN. If you don't have a card, bring a W-2 or pay stub that shows your full number. If you are not eligible for an SSN, you must bring a letter from the SSA stating ineligibility.

Proof of Missouri Residency

Bring 2 of these:

  • Utility bill (electric, water, gas) in parent's name
  • Bank statement
  • School transcript or report card
  • Lease or mortgage agreement
  • Parent's Missouri driver license (if address matches)

Missouri generally requires two different documents to prove residency if you want a REAL ID. For a standard license, one is often enough, but bringing two is safer to avoid a second trip.

Parental / Guardian Consent (applicants under 18)

  • Parent or guardian must sign the application form in person at the office
  • Parent/guardian must provide their own proof of identity
  • If parents are divorced, the custodial parent usually signs
  • Legal guardians must bring court documents proving guardianship
  • Foster parents must bring specific foster care documentation

A parent or legal guardian MUST be present with you to sign the application. They cannot sign it at home and bring it back; they have to do it right there at the counter.

Fees & Payment

$3.50

This fee covers the learner's permit. If you fail the vision or written test, you may need to pay a small re-test fee depending on the office, but the permit fee itself is low.

Application for Missouri Driver License or Non-Driver ID Card (Form DOR-100)

Do NOT sign the form until you are standing in front of the license examiner; they will watch you sign it.

What NOT to Bring

  • Your cell phone (turn it off or leave it in the car)
  • Large bags or backpacks (storage is limited)
  • Friends or siblings who aren't applying
  • Food or drinks
  • Your own pen (use the one provided)
  • Photocopies of documents (bring originals)
  • Pets

At the Office

  • The vision test is first; if you wear glasses, bring them
  • The written test is computer-based; read questions carefully
  • If you fail the written test, you usually have to wait 24 hours to retake it
  • You will get an interim paper permit immediately; the hard plastic card arrives by mail in a few weeks
  • Keep your interim permit with you every time you drive
