What to Bring to the Nevada DMV for Your Permit Test

Don't get sent home. Here's the complete checklist — documents, fees, forms, and what to leave at home.

Before You Go

  • Schedule an appointment online to skip the longest lines
  • Double-check that your parent knows the exact time and has their ID ready
  • Bring your study guide or practice tests to review while waiting
  • Arrive at least 15 minutes early to check in
  • Make sure your eyes are rested; you will take a vision test first
  • Verify your address matches exactly on all your residency documents
  • Eat a good breakfast so you don't get hangry during the test

Required Documents

Proof of Identity (Primary Document)

  • U.S. Passport or Passport Card
  • Certified U.S. Birth Certificate
  • Permanent Resident Card (Green Card)
  • Consular Report of Birth Abroad
  • Certificate of Naturalization or Citizenship

Bring the original document only; Nevada does not accept photocopies or digital versions on your phone for the primary ID.

Proof of Social Security Number

  • Social Security Card
  • W-2 Form
  • SSA-1099 Form
  • Pay Stub with full name and SSN
  • Military ID with SSN

If you don't have a card, bring a W-2 or pay stub that clearly shows your full name and number. Nevada generally requires proof of your number even if you don't have the physical card.

Proof of Nevada Residency

Bring 2 of these:

  • Utility bill (electric, water, gas) in parent's name
  • Bank statement with current address
  • Rental or lease agreement signed by parent
  • Parent's Nevada driver's license or ID
  • School report card or transcript with current address

Nevada requires two different documents to prove residency. Both must show your (or your parent's) current Nevada address.

Parental / Guardian Consent (applicants under 18)

  • Parent or legal guardian must sign the application in person at the DMV
  • Parent must present their own valid photo ID
  • If parents are divorced, the custodial parent usually signs, but check specific custody orders
  • Legal guardians must bring court documents proving guardianship
  • Foster parents must bring specific agency authorization forms

The parent or guardian MUST be physically present with you at the counter to sign; they cannot sign this at home and bring it in.

This fee covers the application and the learner's permit. If you fail the vision or written test, there may be a small retake fee, but the permit fee itself is paid once.

Forms

Application for Driver License or Identification Card (Form DL-180)

Available at: Nevada DMV website or DMV office

You can fill this out online ahead of time to save a few minutes, but DO NOT sign it until you are standing in front of the DMV clerk.

What NOT to Bring

  • Your cell phone (turn it off or leave it in the car; no texting during the test)
  • Large backpacks or bags (storage is limited)
  • Friends or siblings (they can't come inside with you)
  • Pets (unless it's a service animal)
  • Food or drinks (no eating in the testing area)
  • Your own pen (use the one provided)
  • Photocopies of your documents (bring originals only)

At the Office

  • The written test is computer-based and multiple choice; read every question twice
  • If you fail the written test, you can usually retake it the same day for a small fee
  • You will get an interim paper permit immediately if you pass; the real card arrives by mail in 2 weeks
  • The vision test is quick, but wear your glasses or contacts if you need them
  • Listen carefully to the clerk; they will tell you exactly how many hours of driving you need before your next step
