What to Bring to the New Jersey DMV for Your Permit Test

Don't get sent home. Here's the complete checklist — documents, fees, forms, and what to leave at home.

Before You Go

  • Make an appointment online to skip the long lines
  • Fill out the OS-1 form online ahead of time to save time
  • Study the New Jersey Driver Manual thoroughly before going
  • Arrive at least 15 minutes early for your appointment
  • Double-check that all documents are originals and not expired
  • Ask your parent to bring their own ID and be ready to sign
  • Bring a valid form of payment for the $10 fee

Required Documents

Proof of Identity (Primary Document)

  • U.S. Passport or Passport Card
  • Certified U.S. Birth Certificate
  • Permanent Resident Card (Green Card)
  • Certificate of Naturalization or Citizenship
  • Valid Foreign Passport with I-94
  • Military ID

You must bring the original document; photocopies are not accepted. If you want a REAL ID, you need one primary document plus two proofs of residency.

Proof of Social Security Number

  • Social Security Card
  • W-2 Form
  • SSA-1099 Form
  • Pay Stub with full name and SSN
  • Medicare Card

New Jersey requires proof of your SSN. If you don't have one, you must bring a letter from the Social Security Administration stating you are ineligible, but this is rare for citizens.

Proof of New Jersey Residency

Bring 2 of these:

  • Parent's utility bill (electric, gas, water) dated within 90 days
  • Parent's bank statement
  • Parent's mortgage statement or lease agreement
  • School report card or transcript with home address
  • Medical or dental bill
  • Official government mail

For a standard permit, one proof of residency is usually enough if your name is on it, but for a REAL ID, you strictly need two different documents showing your NJ address. Since teens often don't have bills in their name, a parent's document plus a school document usually works.

Parental / Guardian Consent (applicants under 18)

  • Parent or legal guardian must sign the application form in person at the MVC office
  • If the parent cannot be present, a notarized 'Affidavit of Identity' (Form OS-1) may be required in specific cases, but appearing together is the standard rule
  • The parent must bring their own valid photo ID
  • Legal guardians must bring court documents proving guardianship
  • Foster parents must bring specific foster care documentation

New Jersey is strict: the parent or guardian usually has to be there with you to sign the form. If you are a foster child, bring your case worker's contact info and documentation.

Fees & Payment

$10

This $10 fee covers the learner's permit application. If you fail the written test, you must pay another $10 to retake it. The fee for the actual driver's license later is $24.

Forms

Application for Driver License or Non-Driver ID Card (Form OS-1)

Available at: MVC website (nj.gov/mvc) or at the office

Do NOT sign the form until you are standing in front of the MVC agent; they need to witness your signature.

What NOT to Bring

  • Your cell phone (you must turn it off and put it away during the test)
  • Friends or siblings (they cannot wait inside the testing area)
  • Pets (unless they are service animals)
  • Food or drinks (no eating in the testing room)
  • Your own pen (use the one provided by the MVC)
  • Photocopies of documents (they must be originals)
  • Backpacks or large bags (storage is limited)

At the Office

  • The written test is computer-based and multiple choice
  • You get 50 questions and need 80% (40 correct) to pass
  • If you fail, you can retake it the same day for another $10
  • Once you pass, you get an interim paper permit immediately
  • Your plastic permit card will arrive by mail in about 2 weeks
