What to Bring to the New York DMV for Your Permit Test

Don't get sent home. Here's the complete checklist — documents, fees, forms, and what to leave at home.

Before You Go

  • Book an appointment online to skip the long lines
  • Fill out Form MV-44 online beforehand
  • Arrive at least 15 minutes early for check-in
  • Bring your study guide or practice test app on your phone (but turn it off inside)
  • Make sure your parent/guardian knows exactly when you are going
  • Double-check that all documents are originals, not photocopies
  • Eat a good breakfast so you're focused for the test

Required Documents

Proof of Identity (Primary Document)

  • U.S. Passport or Passport Card
  • Certified U.S. Birth Certificate
  • Permanent Resident Card (Green Card)
  • Foreign Passport with valid U.S. Visa
  • Consular Report of Birth Abroad

You must bring the original document; photocopies are not accepted. If you want a REAL ID, you need one primary document plus two proofs of residency.

Proof of Social Security Number

  • Social Security Card
  • W-2 Form
  • SSA-1099 Form
  • Paystub with full SSN
  • Non-SSA-issued document with full SSN

If you don't have a card, you must bring a letter from the Social Security Administration stating you've never been issued one. New York does not waive this requirement.

Proof of New York Residency

Bring 2 of these:

  • Utility bill (electric, gas, water) in parent's name
  • Bank statement
  • School report card or transcript
  • Lease or mortgage statement
  • Parent's driver's license (if address matches)

New York requires 2 proofs of residency for a REAL ID. For a standard license, 1 proof is usually enough, but bringing 2 is safer to avoid delays.

Parental / Guardian Consent (applicants under 18)

  • Form MV-45 (Parent/Guardian Consent Form)
  • Parent must sign the form in front of the DMV clerk or have it notarized
  • Parent or guardian must appear in person with you
  • If parents are divorced, the custodial parent must sign
  • Legal guardians must bring court documents proving guardianship

If you are in foster care, the foster parent or agency representative must sign, and you may need specific court documentation.

Fees & Payment

$12.50

This fee covers the permit application and the written test. If you fail the written test, you must pay a new fee to retake it.

You can fill this out online to save time, but do NOT sign it until you are at the DMV counter in front of the clerk.

What NOT to Bring

  • Large backpacks or bags (storage is limited)
  • Friends or siblings (they can't come inside)
  • Pets (unless service animals)
  • Food or drinks
  • Your own pen (they provide them)
  • Expensive jewelry or electronics
  • Fake IDs or expired documents

At the Office

  • The written test is computer-based and takes about 20-30 minutes
  • You need 14 out of 20 questions correct to pass
  • If you fail, you can usually retake it the same day for an extra fee
  • Once you pass, you get an interim permit immediately; the plastic card arrives by mail in 2 weeks
  • Listen carefully to the clerk's instructions on driving restrictions for your permit
